You'll monitor patient conditions, provide appropriate nursing interventions, manage medications, educate caregivers, document services, and help coordinate an individualized plan of care focused on patient comfort.

VyTalent Solutions is offering a new opportunity for a Registered Nurse (RN) - Hospice seeking their next contract assignment. This 13-week position is based in a Home Health setting and is suited for nurses who value compassionate care and meaningful patient interactions.

Responsibilities:

• Conduct comprehensive hospice patient assessments in home or facility settings.
• Manage symptom control and medication titration, including comfort medications like Versed and Dilaudid.
• Collaborate effectively with interdisciplinary teams to develop and implement individualized care plans.
• Document patient care in real-time, ensuring accurate and timely updates in medical records.
• Educate families and caregivers on end-of-life care, comfort measures, and treatment goals.
