• Pay Range: $2295.60 - $2410.38
• Schedule: 8am-5pm
• Contract Length: 13 weeks

Responsibilities:

• Provide comprehensive nursing care to home health patients in a designated geographic area.
• Perform patient assessments, wound care, and medication administration while managing chronic conditions such as CHF, COPD, CVA, and diabetes.
• Document treatments accurately using systems like EPIC, HCHB, OASIS, and Meditech.
• Educate patients and caregivers on health management and safety protocols.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
