Job Details:
Job Title: Physical Therapist – Home Health
Location: Durham, NC
Setting: Home Health
Pay Range: $2,126.20 – $2,232.51
Schedule: 07:00 – 15:00 (Days)
Contract Length: 13 Weeks
Candidate Type: Local and traveler friendly
Requirements: Valid Physical Therapist license in North Carolina; 3 years of experience as a PT; at least 2 years in home health; willing to obtain state licensure
Responsibilities:
- Deliver one-on-one, high-touch care to patients in their homes
- Conduct skilled patient assessments and create individualized care plans
- Collaborate with families and interdisciplinary providers to improve patient outcomes
- Apply clinical expertise to support patient recovery and well-being
