Job Title: Physical Therapist – Home Health
Location: Durham, NC
Setting: Home Health
Pay Range: $2,126.20 – $2,232.51
Schedule: 07:00 – 15:00 (Days)
Contract Length: 13 Weeks
Candidate Type: Local and traveler friendly
Requirements: Valid Physical Therapist license in North Carolina; 3 years of experience as a PT; at least 2 years in home health; willing to obtain state licensure
Responsibilities:

Deliver one-on-one, high-touch care to patients in their homes
Conduct skilled patient assessments and create individualized care plans
Collaborate with families and interdisciplinary providers to improve patient outcomes
Apply clinical expertise to support patient recovery and well-being
