Job Title: Case Manager | RN – Registered Nurse
Location: Asheville, NC
Setting: Hospital
Pay Range: 2297.60 – 2412.48
Schedule: 08:00 – 16:30
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Work collaboratively with RNCM; Documentation Skills; Communication Skills; Ability to Shift Attention Between Tasks; Ability to Build Trust with Team/Patients and Family
Responsibilities:

• Provide case management services to patients within the hospital setting.
• Collaborate with nursing staff and other healthcare professionals to develop patient care plans.
• Document patient interactions accurately using CERNER, NATE, and ENSOCARE systems.
• Assist in coordinating patient discharge plans and follow-up care.
• Support hospital guidelines and policies while maintaining high standards of patient care.
