Job Details:

Job Title: Home Health RN: Case Manager
Location: Gainesville, GA
Setting: Home Health
Pay Range: 2155.00 – 2262.75
Schedule: M – F 8am to 5pm; On call 1 weekend/month and occasional call 1 day after hrs call during the week days
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: RN; Associates or Bachelor’s Degree in Nursing or related field; Current, unrestricted state Registered Nurse license; Ability to analyze complex medical information and make rational decisions; Case Management certification

Responsibilities:

• Perform comprehensive assessments of high-risk members and continuously reassess their care needs.
• Collaborate with primary care providers to implement individualized, comprehensive care plans.
• Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
• Engage specialty resources and arrange appropriate levels of care to support optimal outcomes.
• Maintain detailed records of clinical, functional, and fiscal outcomes while advocating for patient privacy and confidentiality.
