Job Details:

Job Title: Case Manager Registered Nurse

Location: Chesapeake, VA

Setting: Hospital

Pay Range: 2444.40 – 2566.62

Schedule: 08:00 – 16:30

Contract Length: 13 weeks

Candidate Type: Traveler

Requirements: Active Virginia or Compact RN license in hand at the time of submission; no exceptions. Must be proficient in Epic; no exceptions. Recent experience in an adult acute care hospital setting with discharge planning. Minimum 5 years of recent acute care Case Management experience (hospital setting). Less than a 6-month gap since their last acute care Case Management role. Experience in a Level I, II, or III acute care hospital. Previous patient census of approximately 15-25 patients. Rated 4/5 or 5/5 across all Case Management functions, including knowledge of CMS regulations. Epic proficiency is required (no exceptions). Previous travel experience strongly preferred. First-time travelers will only be considered if they have extensive acute care Case Management experience. Acute care discharge planning. Utilization Review (UR) experience required. Ability to effectively manage a high-volume, fast-paced patient population. BLS through the American Heart Association.

Responsibilities:

• Manage discharge planning for adult patients in a hospital setting.
• Conduct utilization reviews to ensure appropriate patient care.
• Collaborate with healthcare team to coordinate patient care plans.
• Prioritize and effectively manage a high-volume, fast-paced patient caseload.
• Maintain compliance with CMS regulations and hospital policies.
