Job Overview

As a Registered Nurse (RN) - Home Health, you will deliver high-quality skilled nursing care directly to patients within their home environments. In this role, you will evaluate patient health conditions, administer prescribed treatments and medications, and execute individualized care plans aimed at improving health outcomes and fostering independence.

Collaborating closely with patients, family members, and interdisciplinary care teams, you will ensure seamless continuity of care through effective communication and thorough documentation. Additionally, you will educate caregivers on safety protocols and treatment requirements while managing clinical evaluations in accordance with established standards.

• Candidate Type: Local or Travel
• Requirements: 1 year of Medicare certified Home Health experience and HCHB EMR experience

Responsibilities

• Provide skilled nursing care to patients in the home health setting while following established care plans.
• Complete thorough patient assessments and document findings, interventions, and patient responses.
• Communicate effectively with patients, families, and the care team to support continuity of care.
• Educate patients and caregivers on care needs, medications, and safety considerations as appropriate.
• Maintain accurate, timely documentation in accordance with assignment and facility standards.
