Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans.
• Collaborate with interdisciplinary teams to coordinate patient care.
• Educate patients and families about health management and safety.
• Manage case documentation, including familiarity with home health EMR systems.
• Ensure compliance with healthcare regulations and documentation standards.
