Responsibilities:
- Conduct comprehensive hospice patient assessments in home or facility settings.
- Manage symptom control and medication titration, including comfort medications like Versed and Dilaudid.
- Collaborate effectively with interdisciplinary teams to develop and implement individualized care plans.
- Document patient care in real-time, ensuring accurate and timely updates in medical records.
- Educate families and caregivers on end-of-life care, comfort measures, and treatment goals.
