Job Title: RN – Home Care Case Mgmt
Location: Mebane, NC
Setting: Home Health
Pay Range: 2136.95 – 2243.80
Schedule: 07:00 – 15:00 (Day, D/N, Rotating)
Contract Length: 13 Weeks
Candidate Type: Travel
Requirements:
* 2 years of experience as an RN
* Valid RN license
* Be willing to obtain NC licensure
* Experience with OASIS and Homecare Homebase EMR systems
Responsibilities:
- Conduct OASIS assessments, care planning, and discharge planning for home health patients
- Monitor patient progress and provide wound care within the home setting
- Collaborate with interdisciplinary teams to coordinate patient care
- Document patient encounters accurately using Homecare Homebase or similar EMR systems
- Ensure compliance with licensure and certification requirements throughout the assignment
