Job Details:
Job Title: RN – Home Care Case Mgmt
Location: Mebane, NC
Setting: Home Health
Pay Range: 2136.95 – 2243.80
Schedule: 07:00 – 15:00 (Day, D/N, Rotating)
Contract Length: 13 Weeks
Candidate Type: Travel
Requirements:
* 2 years of experience as an RN
* Valid RN license
* Be willing to obtain NC licensure
* Experience with OASIS and Homecare Homebase EMR systems
Responsibilities:

Conduct OASIS assessments, care planning, and discharge planning for home health patients
Monitor patient progress and provide wound care within the home setting
Collaborate with interdisciplinary teams to coordinate patient care
Document patient encounters accurately using Homecare Homebase or similar EMR systems
Ensure compliance with licensure and certification requirements throughout the assignment
