Responsibilities:
- Deliver one-on-one, high-touch care to patients in their homes.
- Conduct skilled assessments and develop individualized care plans.
- Collaborate with patients’ families and interdisciplinary teams to improve outcomes.
- Perform therapy sessions according to plan and document progress accurately.
