- Schedule: 8-Hour Days, 07:00:00-15:00:00
- Contract Length: 13 weeks

Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans.
- Collaborate with interdisciplinary teams to coordinate patient care.
- Educate patients and families about health management and safety.
- Manage case documentation, including familiarity with home health EMR systems.
- Ensure compliance with healthcare regulations and documentation standards.
