Job Details:

Job Title: RN – Home Care Case Mgmt
Location: Baltimore, MD
Setting: Home Health
Pay Range: $2,142.81 – $2,249.95
Schedule: 8-Hour Days, 07:00:00-15:00:00
Contract Length: 13 weeks
Candidate Type: Local and traveler friendly

Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans.
Collaborate with interdisciplinary teams to coordinate patient care.
Educate patients and families about health management and safety.
Manage case documentation, including familiarity with home health EMR systems.
Ensure compliance with healthcare regulations and documentation standards.
