Job Details:
Job Title: Case Manager | RN – Registered Nurse
Location: Port Charlotte, FL
Setting: Hospital
Pay Range: 1826.49 – 1917.81
Schedule: 08:00 – 16:30 (every other weekend and Holidays in rotation with Hospital employees)
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Hospital Case Management Experience
Responsibilities:
- Provide case management support within the hospital setting.
- Coordinate patient care activities with the interdisciplinary care team.
- Manage assigned patient caseloads in accordance with department expectations.
- Complete accurate and timely documentation in the electronic medical record.
- Demonstrate strong attention to detail, time management, and multitasking skills.
