Pay Range: $2,062.50 – $2,165.63

Contract Length: 13 weeks

Requirements: Homecare Homebase Software Experience; OASIS Experience; Current state license as a Registered Nurse; Proof of current CPR certification; Current driver’s license; Reliable transportation and insurance; 1-2 years experience as a Registered Nurse in a clinical care setting; 2+ years Home health experience; Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
Responsibilities:
- Administer skilled nursing care to clients of all ages in their place of residence.
- Assess patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Educate patients and families regarding care, diet, medications, home safety, and self-care practices.
- Coordinate care with patients, families, interdisciplinary team members, referring agencies, physicians, and other healthcare professionals.
