Job Details:
Job Title: Case Manager
Location: Doylestown, PA
Setting: Hospital
Pay Range: 2355.40 – 2473.17
Schedule: 08:00 – 16:30; Monday – Friday; start time ranges from 8a-9a for 8.5 hours; weekends required once per every 4 weeks
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: 2+ years of acute care Case Manager experience; Non acute disciplines will not be considered; Active PA RN license; BSN
Responsibilities:
- Coordinate case management activities in a hospital setting.
- Support patient care planning and coordination across the care continuum.
- Collaborate with healthcare professionals to facilitate appropriate patient care.
- Monitor patient needs and help coordinate services throughout the care process.
- Maintain accurate case management documentation and communication.
