Job Details:
Job Title: Hospice | LPN – Licensed Practical Nurse
Location: Peachtree City, GA
Setting: Home Health
Pay Range: $1,462.23 – $1,535.34
Schedule: Monday-Friday 8am-5pm (on-call as needed by rotation)
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Current Nursing License in the Practicing State; Valid Drivers License; Current CPR Certification; Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting; Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing; Proof of current Hepatitis consent/declination and TB vaccination record; Ability to use electronic medical charting and time-keeping software (Homecare Homebase); Reliable transportation and current auto liability insurance coverage; Must read, write and speak fluent English; Must have good and regular attendance

Responsibilities:

• Administer skilled nursing care to patients in their place of residence in accordance with the established plan of care.
• Assist the RN in providing nursing services and identifying patients’ physical, psycho-social, and environmental needs.
• Coordinate and implement patient care with the RN, interdisciplinary team, patient, and family.
• Document care, interventions, outcomes, and communication in the clinical record.
• Assist patients and families with self-care techniques, admissions, discharges, transfers, and specialized end-of-life care duties.
