Job Title: Home Health – Registered Nurse Clinical Manager

Pay Range: $2,504.87 – $2,630.11

Contract Length: 13 weeks
Candidate Type: Traveler or local
Requirements: Must possess a valid state driver’s license and automobile liability insurance Must be able to drive an automobile in all types of weather conditions Must have an RN license in the State of employment. Must have management/supervisory clinical experience.
Responsibilities:
- Ensure the client’s plan of care is individualized and appropriately executed, with reassessment by licensed professionals as needed.
- Review and verify evaluation packets, OASIS, 485s, and other documentation for accuracy.
- Follow up on clinical assessments, orders, and documentation deficiencies to ensure compliance and timely patient care.
- Participate in patient visits, on-call rotations, and multidisciplinary team meetings as needed.
- Manage referral intake, schedule assessment visits, and coordinate with physicians for orders and approvals.
- Responsible for Referral Intake and Management to ensure that clients receive assessment visits and that these visits are scheduled and performed timely.
- Lead weekly Case Conference and address recert / discharge decisions based on Case Conference findings.
- Review and approve Case Conference coordination notes and attach to Episode Detail Report.
