Job Title: Case Manager | RN – Registered Nurse
Location: Englewood, CO
Setting: Hospital
Pay Range: 1856.51 – 1949.34
Schedule: 08:00 – 16:30 (Travel)
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Minimum 1-year current Case Management RN experience
Responsibilities:
- Manage patient discharge planning assessments and processes.
- Coordinate with multidisciplinary teams to ensure smooth discharge workflows.
- Provide case management services aligned with unit policies and patient needs.
- Document assessment and care plans accurately within EMR systems such as Meditech and CarePort.
- Communicate effectively with patients, families, and healthcare teams to facilitate care transitions.
