Job Title: Case Manager – Registered Nurse
Location: Fort Myers, FL
Setting: Hospital
Pay Range: 1735.68 – 1822.46
Schedule: 11:00 – 19:00
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: BLS; prior travel experience; 2 years of CM experience; acute care experience; FL/Compact nursing license; recent Nursys profile; 5 years of experience or degree accreditation on profile
Responsibilities:
- Provide case management services within the emergency department for readmission initiatives.
- Support acute care patient needs through care coordination and resource management.
- Collaborate with medical teams to facilitate patient discharge planning and follow-up.
- Function as a core staff member, including floating to sister campuses as needed.
- Adhere to all documentation deadlines and profile requirements, including demographic information.
