Pay Range: 1856.35 – 1949.17
Schedule: 08:00 – 17:30
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: BLS; Active Maryland RN license; Minimum 2 years of hospital inpatient RN Case Management/discharge planning experience; Med-Surg and/or Critical Care discharge planning experience; Strong inpatient discharge planning experience; Experience coordinating discharge needs, resources and transitions of care; Must be able to work independently after approximately 3 days of orientation.
Responsibilities:
- Manage discharge planning and patient transitions in the Emergency Department and medical inpatient units.
- Support adult and geriatric patients, ensuring safe and effective care coordination.
- Collaborate with Social Work to coordinate patient discharges and transitions of care.
- Support approximately 1.5-2 units during crisis staffing in partnership with Case Management and Social Work.
- Ensure compliance with hospital policies and clinical protocols using Cerner charting system.
