Job Title: Case Manager Registered Nurse

Pay Range: 1856.35 – 1949.17
Schedule: 08:00 – 17:30
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Active Maryland RN license; BLS; minimum 2 years of hospital inpatient RN Case Management/discharge planning experience; Med-Surg and/or Critical Care discharge planning experience; strong inpatient discharge planning experience; experience managing medically complex Med-Surg and Critical Care patients; ability to work independently after approximately 3 days of orientation; experience coordinating discharge needs, resources, and transitions of care.
Responsibilities:
- Support inpatient discharge planning and transition of care for adult and geriatric patients in a hospital setting.
- Partner with Social Work to coordinate patient discharges and ensure smooth patient transitions.
- Manage medically complex patients, including those in Med-Surg and Critical Care units.
- Support coverage of Emergency Department and medical inpatient units, with flexibility to float between units as needed.
- Work independently after a brief orientation, maintaining focus on patient needs and resource coordination.
