Job Title: Hospice Registered Nurse
Location: Onley, VA
Setting: Home Health
Pay Range: 2296.80 – 2411.64
Schedule: 08:00 – 16:30
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Home health hospice case manager must have hospice exp in the home health setting
Responsibilities:
- Provide hospice nursing care to patients in a home health setting
- Coordinate patient care plans in collaboration with families and healthcare teams
- Document patient assessments, interventions, and progress accurately
- Support patients and families through end-of-life care processes
- Ensure compliance with hospice regulations and standards
