Pay Range: 2108.80 – 2214.24

Contract Length: 13 weeks.

Requirements: Homecare Homebase Software Experience – Required
OASIS Experience – Required
Current state license as a Registered Nurse – Required
CHPN certification – Preferred
Proof of current CPR certification – Required
Current driver’s license – Required
Must have reliable transportation and insurance – Required
1-2 years experience as a Registered Nurse in a clinical care setting – Required
2+ years Home health experience – Required
Education: Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program – Required; Bachelors of Science in Nursing – Preferred
Responsibilities:
- Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Educate patients and families regarding care, medications, and safety practices.
- Provide clinical directions to Home Health Aides and Licensed Vocational Nurses to ensure quality service.
- Coordinate referrals and ensure compliance with third-party payer requirements.
