Pay Range: 2063.63 – 2166.81

Contract Length: 13 weeks.

Requirements: Homecare Homebase Software Experience – Required, OASIS Experience – Required, Current state license as a Registered Nurse – Required, CHPN certification – Preferred, Proof of current CPR certification – Required, Current driver’s license – Required, Must have reliable transportation and insurance – Required, 1-2 years experience as a Registered Nurse in a clinical care setting – Required, 2+ years Home health experience – Required, Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program – Required, Bachelors of Science in Nursing – Preferred.
Responsibilities:
- Assess home health patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
- Implement, develop, and evaluate the Plan of Care to ensure quality and continuity of care.
- Educate patients and families regarding care, medications, and safety practices.
- Supervise and provide clinical directions to home health aides, LPNs, and LVNs.
- Perform on-call responsibilities and coordinate referrals to appropriate agencies.
