Job Title: Home Health | RN – Registered Nurse
Location: Bradenton, FL

Pay Range: 2063.63 – 2166.81

Contract Length: 13 weeks.
Candidate Type: Traveler
Requirements: Homecare Homebase Software Experience – Required, OASIS Experience – Required, Current state license as a Registered Nurse – Required, CHPN certification – Preferred, Proof of current CPR certification – Required, Current driver’s license – Required, Must have reliable transportation and insurance – Required, 1-2 years experience as a Registered Nurse in a clinical care setting – Required, 2+ years Home health experience – Required, Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program – Required, Bachelors of Science in Nursing – Preferred.

Responsibilities:

Assess home health patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
Implement, develop, and evaluate the Plan of Care to ensure quality and continuity of care.
Educate patients and families regarding care, medications, and safety practices.
Supervise and provide clinical directions to home health aides, LPNs, and LVNs.
Perform on-call responsibilities and coordinate referrals to appropriate agencies.
