Job Details:

Job Title: Home Health RN
Location: Buford, GA
Setting: Home Health
Pay Range: 2100-2300
Schedule: Monday-Friday 8-5
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: 1 years of Medicare certified Home Health experience as well as HomeCare HomeBase EMR and OASIS experience.

Responsibilities:

• Provide skilled nursing care to patients in the home setting while supporting individualized care plans.
• Assess patient status, document clinical findings, and communicate updates to the care team as needed.
• Educate patients and caregivers on care routines, safety considerations, and ongoing health management.
• Coordinate care and help ensure timely follow-up with appropriate clinical and support resources.
• Maintain accurate, timely documentation in alignment with assignment expectations and quality standards.
