Pay Range: 2055.80 – 2158.59

Contract Length: 13 weeks

Requirements:
- Homecare Homebase Software Experience – Required
- OASIS Experience – Required
- Current state license as a Registered Nurse – Required
- CHPN certification – Preferred
- Proof of current CPR certification – Required
- Current driver’s license – Required
- Must have reliable transportation and insurance – Required
- 1-2 years experience as a Registered Nurse in a clinical care setting – Required
- 2+ years Home health experience – Required
Responsibilities:
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Educate patients and their families regarding care, medication, and safety practices.
- Supervise and provide clinical directions to home health aides and staff to maintain quality services.
- Coordinate referrals to appropriate agencies and monitor compliance with third-party payer requirements.
