Requirements:

Homecare Homebase Software Experience – Required
OASIS Experience – Required
Current state license as a Registered Nurse – Required
CHPN certification – Preferred
Proof of current CPR certification – Required
Current driver’s license – Required
Must have reliable transportation and insurance – Required
1-2 years experience as a Registered Nurse in a clinical care setting – Required
2+ years Home health experience – Required

Responsibilities:

Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
Educate patients and their families regarding care, medication, and safety practices.
Supervise and provide clinical directions to home health aides and staff to maintain quality services.
Coordinate referrals to appropriate agencies and monitor compliance with third-party payer requirements.
