Job Details:
Job Title: Hospice RN Case Manager
Location: Atlanta, GA (3 territory options Brookhaven/Sandy Springs; Cobb/Cherokee Counties; Buckhead/Downtown ATL including homes and assisted living facilities)
Setting: Home Health
Pay Range: $92,000-$97,000 depending on experience (to be negotiated with facility)
Schedule: Monday-Friday 8am-5pm (on-call as needed on rotational/volunteer basis)
Candidate Type: Local seeking permanent full-time employment
Requirements: 2 years of Hospice experience, HCHB EMR experience preferred.
Responsibilities:
- Provide comprehensive hospice nursing care to patients in a home health setting.
- Coordinate patient care plans and communicate with multidisciplinary teams.
- Monitor patient progress and adjust care as necessary.
- Ensure compliance with all regulatory and safety guidelines.
- Educate patients and families about hospice services and comfort measures.
