Pay Range: 2022.80 – 2123.94

Responsibilities:
- Assess home health and hospice patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and their families regarding care, medications, diet, and home safety.
- Supervise and provide clinical directions to HHA/STNA/LVN team members to ensure quality service.
- Perform on-call responsibilities and provide on-call services to patients/families as assigned.
