Job Details:

Job Title: CWHH-RN Home Health

Location: Houma, LA

Setting: Home Health

Pay Range: 2022.80 – 2123.94

Schedule: Discuss with recruiter.

Contract Length: 13 weeks.

Candidate Type: Travel and traveler friendly.

Requirements: Connect with your recruiter for role-specific requirements.

Responsibilities:

Assess home health and hospice patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
Educate patients and their families regarding care, medications, diet, and home safety.
Supervise and provide clinical directions to HHA/STNA/LVN team members to ensure quality service.
Perform on-call responsibilities and provide on-call services to patients/families as assigned.
