Responsibilities:

Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.

Develop, implement, and evaluate the Plan of Care to ensure quality and continuity of care.

Educate patients and families regarding care, medications, and safety measures, promoting self-care practices.

Supervise and direct home health aides/stna/lvn to maintain quality of service.

Perform on-call responsibilities and provide on-call services to patients and families as assigned.
