Responsibilities:
- Provide physical therapy assessments and develop treatment plans for patients in a Long Term Care setting.
- Implement therapy sessions to improve patient mobility, strength, and overall well-being.
- Document patient progress, goals, and outcomes accurately according to facility protocols.
- Collaborate with healthcare team members to coordinate patient care.
- Ensure compliance with all regulatory and safety standards.
