Job Title: Case Manager Registered Nurse

Pay Range: 1856.35 – 1949.17
Schedule: 08:00 – 17:30
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Active Maryland RN license; minimum 2 years of hospital inpatient RN Case Management/discharge planning experience; Med-Surg and/or Critical Care discharge planning experience; BLS; strong inpatient discharge planning experience; able to work independently after limited orientation; experience coordinating discharge needs, resources, and transitions of care.
Responsibilities:

• Support adult and geriatric inpatient populations in the Emergency Department and medical inpatient units.
• Partner with Social Work and other healthcare professionals to coordinate discharge planning and patient transitions.
• Manage medically complex Med-Surg and Critical Care patients, ensuring safe and effective discharge processes.
• Support approximately 1.5-2 units during crisis staffing, supporting multiple inpatient units as needed.
• Chart patient progress and discharge details using Cerner system, maintaining accurate documentation.
