Job Title: CWHH-Physical Therapist

Location: Bardstown, KY

Setting: Home Health

Pay Range: 2492.00 – 2616.60

Schedule: Discuss with recruiter.

Contract Length: 13 weeks.

Candidate Type: Traveler

Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver’s license; Must have reliable transportation and insurance; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting

Responsibilities:

• Perform clinical assessments to identify patient needs and determine appropriateness of therapy.
• Develop, review, and revise patient care plans and progress notes, collaborating with physicians and interdisciplinary team members.
• Plan and provide treatments for neuromuscular, neurological, and orthopedic disorders based on medical referral and evaluations.
• Instruct patients and families on transfer techniques, use of equipment, and rehabilitation devices safely.
• Educate patients and families regarding therapeutic procedures tailored to their age, condition, culture, and learning readiness.
