Job Details:

Responsibilities:

Provide physical therapy assessments and develop treatment plans for patients in a Long Term Care setting.
Implement therapy sessions to improve patient mobility, strength, and overall well-being.
Document patient progress, goals, and outcomes accurately according to facility protocols.
Collaborate with healthcare team members to coordinate patient care.
Ensure compliance with all regulatory and safety standards.
