Job Details:

The Home Health RN role provides a dedicated 13-week contract opportunity with compensation ranging from $2,100 - $2,300. The position is based in a Home Health setting, follows an 08:00 - 17:00 schedule, and is available to both local and traveler candidates.

Applicants must have a minimum of 1 year of Medicare-certified Home Health experience, plus 1 year of experience with HomeCare HomeBase (HCHB) EMR and OASIS. This role is designed for experienced RNs seeking a specialized Home Health assignment.

Responsibilities:

• Provide comprehensive nursing care to home health patients.
• Assess patient needs and develop individualized care plans.
• Monitor patient progress and update healthcare team accordingly.
• Educate patients and their families on health management and disease prevention.
• Document patient care activities accurately and timely.
