Job Overview:

The Physical Therapist - Home Health is responsible for providing rehabilitation care directly in patients' homes. Duties may include completing physical therapy assessments, establishing treatment goals, delivering therapeutic interventions, monitoring functional progress, maintaining clinical records, and coordinating care with other healthcare professionals.

Job Details:

The Physical Therapist (PT) - Home Health will deliver rehabilitation services to patients in their homes throughout a 13-week assignment. This role offers $2,391.40 - $2,510.97 with a schedule from 7:00 AM to 3:00 PM. Local candidates are welcome, while travel stipends are not available for local assignments.

The PT will support patient mobility, recovery, and functional needs through skilled therapy services in the home setting. Traveler candidates are also welcome, with travel reimbursement available. For specific qualifications and assignment requirements, candidates should connect with their recruiter.

Responsibilities:

• Deliver one-on-one, high-touch care to patients in their homes.
• Conduct skilled nursing visits and perform assessments.
• Create and manage individualized care plans.
