Responsibilities:

• Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Obtain data on psychological, social, spiritual, and cultural factors affecting patient/family health and incorporate into the POC.
• Maintain accurate, up-to-date patient records, documenting visits, contacts, problems, plans, and changes.
• Provide care while following infection control measures to protect staff and patients.
• Educate patients and families on care, medication, diet, and safety, utilizing appropriate teaching methods.
• Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality service.
• Assess and reassess pain, utilizing appropriate management techniques, and educate about pain control.
• Arrange referrals to appropriate agencies when necessary.
• Monitor cases for compliance with third-party payer requirements.
• Perform on-call responsibilities and provide services to patients/families as assigned.
• Communicate effectively with patients, families, team members, and healthcare professionals.
• Attend interdisciplinary team meetings as needed.
• Perform additional duties as assigned or requested.
